Can anyone send me a link or cut and paste of a notarized affidavit,?

I am interested in pursuing my lSW and LCSW but was fired for an ethical violation I feel the agency jumped to conclusions and did not have probable cause. please provide examples. I know the indiana NASW reviews all afidavits
